Milestone’s Community Health Worker (CHW) serves as a trusted link between healthcare systems and the community.
The CHW provides education, outreach and support to help individuals understand health information, navigate healthcare services and adopt healthier behaviors.
Often coming from the communities they serve, they build strong relationships with residents, advocate for community needs and connect people with resources such as preventive care, nutrition programs and social services. Their work helps improve health outcomes and reduce disparities by bringing culturally responsive support directly into communities.
- System Navigation and Resource Connection: Helping individuals navigate healthcare systems, apply for insurance and connect with social services such as food banks, housing support and transportation.
- Outreach and Home Visits: Conducting outreach in the community and home visits to provide Social Detriments Of Health screenings, monitor health conditions and provide in-person support.
- Health Education and Coaching:Â Providing resources for education on healthy behaviors, chronic disease management (e.g., diabetes, heart disease), and prevention.
- Advocacy and Support: Advocating for individuals’ or the community’s needs to providers and policymakers, as well as providing social support and informal counseling.
- Data Collection and Reporting: Gathering information on community health needs and recording progress of people supported to evaluate and improve programs.
- Cultural Liaison: Translating or interpreting for persons we support to ensure health information is understood and culturally appropriate.
