Milestone’s Administrative Service Coordinators (ASCs) help individuals access and stay connected to mental health services by coordinating care and managing the administrative processes that support treatment with less intensive support then Service Coordinators.
In our clinical setting, they assist with intake, service authorizations, liability, benefit assistance, referrals to behavioral health services – along with linkages to community resources, service planning and communication with treatment teams, medication services, engagement & outreach, and coordination of physical health. ASCs also help coordinate transitions in care, such as hospital discharges to the outpatient clinic or a higher level of care as determined.
By managing documentation, records and coordination across systems, they help ensure individuals receive timely, organized, and continuous mental health support.
- Liability & Revenue Verification: Determine financial liability and verify sources of revenue/coverage.
- Benefits Assistance: Help individuals and families apply for and maintain eligible health coverage benefits.
- Behavioral Health Referrals: Connect to outpatient care, service coordination, housing, partial hospitalization, employment supports and justice-related services.
- Service Planning & Coordination: Collaborate with treatment teams and support discharge planning.
- Community Resource Linkages: Connect to transportation, Medical Assistance Transportation Program (MATP), food resources, homelessness services and housing supports.
- Medication Coordination: Assist with prescriptions, delivery and payment through pharmacy benefit programs.
- Engagement & Outreach: Provide appointment reminders, scheduling (including urgent needs), correspondence and community-based outreach to individuals and supports.
- Physical Health Coordination: Facilitate access to dental, vision, primary care, specialty care and lab services.
