Milestone’s Service Coordination department focuses on helping individuals and families access the services and resources they need to improve their overall well-being.
A service coordinator works closely with people supported to assess needs, develop treatment plans and connect them with appropriate programs such as healthcare, housing assistance, education, or social services.
By facilitating communication between providers and ensuring services are delivered effectively, service coordination helps reduce barriers and promotes more seamless, person-centered support.
- Target Populations: Services are tailored for adults with severe and persistent mental illness.
- Core Responsibilities: Coordinators identify individual strengths, needs and psychological triggers, create individualized treatment plans, locate resources, monitor service delivery, provide Social Detriments of Health linkages, and advocate for the individual.
- Independent Living Support: Services include aiding in obtaining affordable housing, securing personal assistance, arranging transportation and fostering community integration.
- Crisis Management & Advocacy: Coordinators offer 24/7/365 service support with hospital diversion, assist with emergency situations, including hospital admissions, discharge planning and accessing behavioral health crisis resources and advocating for clients rights.
- Service Coordination & Linking: Service Coordinators actively linking clients to mental health, physical health, addiction treatment and social services (e.g., housing, food, education, vocational training).
- Case Monitoring: Our team continuously reviews treatment progress, facilitating interdisciplinary team approaches and ensuring continuity of care across providers.
- Support System Building: Educating people we support on their mental health, reducing stigma and helping to build informal support networks, such as family and friends.
- Accessing Benefits: Assisting people we support with applying for social security, insurance and other financial entitlements.
